Open ACOM Health and Log In

Launch ACOM Health (either the desktop application or web portal depending on your setup) and sign in with admin credentials. ACOM Health is used by chiropractic and physical therapy practices — the interface includes both clinical and billing modules.

Tip:

Admin or Office Manager role needed for Reports access. If the Reports section is not visible, contact ACOM Health support.

Navigate to Reports

Click "Reports" in the main navigation menu (top bar or left sidebar depending on your version). The Reports module opens showing categories of available reports.

Open "Patient Reports" → "Patient Demographics"

In the Reports section, click "Patient Reports". Then select "Patient Demographics" or "Patient List" — this is the core patient data export report. Avoid financial or billing reports for this step.

CRITICAL: ACOM's Three Patient Statuses

ACOM Health uses three patient status categories. Active: patient is currently receiving care. Inactive: patient has not been seen recently but the care relationship was not formally closed. Discharged: the care relationship was explicitly terminated by the provider. Select Active + Inactive only for MedMasters campaigns. The "Discharged" status means the provider formally closed the patient relationship — contacting Discharged patients without careful review may have legal implications.

Generate Report

Note:

"Discharged" in ACOM Health means the provider formally ended the patient care relationship — different from simply not having been seen recently. Do not include Discharged patients in marketing outreach without first consulting with MedMasters or your legal/compliance team.

Configure Fields and Generate

Set Patient Status to include Active and Inactive only. Set Date Range = "All Time". Make sure these fields are selected in the output: First Name, Last Name, Cell Phone, Email, Last Visit Date, Patient Status, Primary Insurance, Visit Count. Click "Generate Report".

  • Status: Active + Inactive (do not include Discharged)
  • Date Range: All Time
  • Provider: All Providers
  • Output fields: First Name, Last Name, Cell Phone, Email, Last Visit Date, Patient Status, Insurance, Visit Count
  • Click Generate Report

Export to CSV or Excel

After the report loads, click the "Export" button in the report toolbar. Choose CSV or Excel format. Save the file and upload to MedMasters via the secure link.
