Export Steps
Sign in to CharmHealth
Open ehr.charmtracker.com in a desktop browser. Admin role is required.
Open Reports → Patient List
Click Reports → Standard Reports → Patient List. Filter by Status (All) and Date (All Time).
Export to CSV
Click Run, then click the export icon and choose CSV. The file downloads to your browser.
Send via Secure Upload Link
Upload the file using the secure link your MedMasters account manager provided. Do not email the CSV file directly — patient contact data must be transmitted securely.
Note:
Never send patient data as a plain email attachment. Patient data sent over unencrypted email violates HIPAA. Use only the secure upload link provided by MedMasters.

Known Quirks
CharmHealth has multiple plan tiers — Patient List export is in all tiers, Custom Reports only on Plus/Pro.
Telehealth visits and in-person visits both count in last_visit.
Pro Tips
Use the "Patient Tags" feature pre-export to segment by service line.
Run a parallel Encounter Report if you need granular visit reasons.
