Log In to Practice Fusion

Open your browser and navigate to app.practicefusion.com. Enter your administrator email and password and click "Sign In". Practice Fusion is fully cloud-based.

Tip:

You need Administrator access in Practice Fusion to create and run patient reports. Provider and Staff logins may not have access to the Reports section.

Navigate to Reports → Patient Lists

In the top navigation, click "Reports". The Practice Fusion reports center will open. Look for "Patient Lists" in the left navigation of the reports section and click it.

Create a New Patient List Report

On the Patient Lists screen, look at the "Actions" dropdown or find a button that says "Select New Report" or "Create New Report". Click it to start building a new patient list report. You can also use an existing saved report if one has been configured previously.

Configure Report Columns and Filters

In the report builder, use the arrow selectors to add columns to your report. Add the following columns by clicking them and moving them to the "Selected" side: First Name, Last Name, Phone Number, Email, Last Visit Date, Patient Status. Leave the Status filter set to include All Patients (both Active and Inactive). Click "Run" to generate the report.

  • Use the arrow buttons to add: First Name, Last Name, Phone Number, Email, Last Visit Date, Patient Status
  • Set Status filter to "All Patients" (do not restrict to Active only)
  • Click "Run" to generate the report

Tip:

Practice Fusion's report builder uses a column selector — you must manually add the fields you want. If you don't add "Last Visit Date" and "Patient Status", they will not appear in the export.

Export to Excel

Once the report loads with results, click the "Export to Excel" button that appears near the top of the results. Practice Fusion will generate and download an Excel (.xlsx) file containing all patients that matched your report criteria.

Tip:

If you need a CSV instead of Excel, open the downloaded .xlsx file in Excel and use File → Save As → CSV (Comma delimited).

Send to MedMasters

Upload the Excel file using the secure MedMasters upload link provided by your account manager.

Note:

Do not email patient data. Always use the secure MedMasters upload portal.

Known Quirks

Practice Fusion's report builder requires you to manually add each column — fields are not included by default

Save the report configuration after setting it up the first time — it will be available under "Saved Reports" for future exports without re-configuring
