EMR

Electronic Medical Record — usually a single-practice digital chart system.

EMR (Electronic Medical Record) historically referred to digital charts inside a single practice that didn't share data outside the office. Today most vendors call themselves EHRs even when behaving like EMRs. The term still shows up in vendor marketing and federal documentation.
