PHI

Protected Health Information — health data tied to an identifiable person.

PHI (Protected Health Information) is any health information that can be tied to a specific person — names, addresses, dates of service, diagnoses, etc. PHI is what HIPAA actually protects. If you fully de-identify a record, it's no longer PHI.
